Understanding Keloid Treatment

Keloid treatment is a doctor performed dermatology procedure that works on the size, thickness and appearance of a raised scar. A keloid forms when the skin lays down too much collagen while repairing itself, so the scar builds upward and outward instead of settling flat. Common triggers in Pakistan include ear and nose piercings, acne on the chest and back, chickenpox marks, burns, the BCG vaccination site and surgical wounds. Some people are simply more prone to them, and it often runs in families. Treatment can involve intralesional injections, cryotherapy, silicone and pressure therapy, laser work, or careful surgical removal combined with follow up treatment to reduce the chance of return. The combination is decided after examination.

The doctor takes a proper history: when the scar started, what caused it, whether it itches or hurts, what has already been tried on it, and whether anyone in the family gets the same kind of scarring. Diagnosis before anything is removed

Not every raised lump on the skin is a keloid. A hypertrophic scar, a cyst, a dermatofibroma and other growths can look similar to the naked eye and are managed differently. The doctor examines the borders, surface, firmness and growth pattern, and decides what the lesion actually is. Nothing is removed first and questioned later. This is the line between a clinic and a salon.

Laboratory examination where it is needed

If anything about the lesion looks unusual, for example rapid growth, an irregular surface, bleeding, ulceration or a history that does not fit, a sample is sent for histopathology before treatment continues. Tissue that is surgically removed is also sent to the laboratory. This is routine, careful practice rather than a reason to worry, and the report guides what happens next.

A small earlobe keloid and a wide keloid across the chest are not the same job. Depending on size, site, age of the scar, skin tone and symptoms, the doctor may use intralesional injections, cryotherapy, silicone and pressure therapy, laser work for redness and texture, or excision combined with follow up treatment. What is the difference between a keloid and a normal raised scar?

A hypertrophic scar stays inside the borders of the original wound and often flattens slowly on its own over a year or two. A keloid grows past those borders, onto skin that was never injured, and rarely settles by itself. The two respond to different treatments, so the doctor examines the edges, the site and the history before deciding anything.

Will my keloid come back after treatment?

It can. A keloid reflects the way your skin heals, so the honest word is managed or controlled, not cured. Treatment can flatten the lesion, soften it and settle the itching and tenderness, and review visits keep it in check. Recurrence is most likely when a keloid is simply cut out with nothing done afterwards, which is why follow up care is planned from the first visit.

Is keloid treatment painful?

Injections into keloid tissue sting for a few seconds because the scar is dense, and most patients describe it as bearable rather than severe. Numbing cream, cooling or local anaesthetic is used where it helps. Cryotherapy feels like a sharp cold sensation for a short time. If a lesion is being surgically removed, the area is fully numbed first and you will not feel the procedure itself.

Can I just get the keloid cut off?

Surgery on its own is the approach most likely to disappoint. Cutting out a keloid creates a fresh wound in skin that has already proved it over heals, and the scar often returns larger than before. When excision is genuinely the right choice, the doctor combines it with injections, pressure or other follow up treatment, and the removed tissue is sent to the laboratory for examination.

Is a keloid dangerous, or can it turn into cancer?

A true keloid is benign. The reason a doctor examines it properly is that a small number of other growths can look like a raised scar to the naked eye, and a few of those need different handling. If the shape, growth speed, surface or history looks unusual, or if tissue is removed, it goes to the laboratory. That is standard care, not a cause for alarm.

I have a keloid on my earlobe from a piercing. Can I wear earrings again?

Often yes, but not immediately and not without a plan. Ear keloids are common here because piercing is usually done young and sometimes with unclean equipment. Once the lesion has been treated, the doctor decides when, or whether, the ear can be pierced again. Pressure earrings are frequently advised afterwards to stop the scar tissue building up a second time.

Do home remedies like toothpaste, garlic, lemon or chuna work on keloids?

No, and some cause real harm. Keloid tissue sits deep in the skin, so nothing applied at home reaches it. Caustic pastes, burning and thread tying at a salon or by an untrained person injure the skin further, and injury is exactly what makes keloids grow. Medical grade silicone gel or sheets, used as directed by the doctor, are one of the few home measures that genuinely help.
